In the ascending pathway for sensation of temperature and pain, the first sensory fiber, after entering the spinal cord from the posterior root, immediately synapses with the cell body of the secondary sensory neuron. The fiber of the secondary neuron crosses the right-left midline immediately at the spinal nerve level, and ascends the spinal cord on the contralateral side. This pathway is called the spinothalamic tract.
